Welcome to the Valleyview Health Office website!

It is the respiratory illness and flu season!
Please see the following links regarding flu, covid, rsv and colds. 

Please remember to keep your student home if they have a fever.  They must be 24 hours fever free without using fever reducing medicines to return to school.    Please contact your school's nurses for further questions.

Please be sure to have medication forms in to the health office prior to any class trips. No medication including, but not limited to, Tylenol, Motrin, allergy medication, motion sickness medication and eye drops, will be dispensed without this form. The form is located on the health office website under forms and must be in to the Health Office prior to the day of the trip. Medications MUST be in the original packaging

***YOU MUST GO INTO FORMS AND COMPLETE THE VALLEYVIEW ATHLETIC FORMS TO READ AND  SIGN-OFF  FOR SUDDEN CARDIAC DEATH, CONCUSSION, EYE INJURY, OPIOID AND STUDENT ATHLETE SAFETY ACT IN ORDER TO PARTICIPATE IN AFTER SCHOOL SPORTS.  THESE ARE STATE MANDATED FORMS***

** PRE-PARTICIPATION Sports physical forms are available under the Athletics icon on the VV site. These are the only forms accepted. There are 4 pages, 2 for parents to fill out and 2 for the pediatrician to fill out. DO NOT send in a Universal Health form as it will not be accepted for sports. Physicals are only good for 365 days from the date the physician performed it.

Sports Physical Due Dates:

You must have a Sports Physical form, NOT a Universal health form, to try out for Valleyview sports.(found under the athletics tab on the Valleyview site or under Health Office- Forms)


Fall Sports:Soccer, Cross Country

Physical Due Date: August 4th.  Your physical will NOT BE CLEARED for you to start if turned in after this date.


Winter Sports:Basketball, Wrestling, Cheerleading , Intramural Flag football, Intramural Basketball

Physical Due Date: October 6


Intramural Volleyball

Physical Due Date: December 15


Spring Sports: Baseball, Softball

Physical Due Date: February 9

 

** Incoming 6th graders MUST have their 11year old vaccines, T-dap and Menactra/ Meningitis, into the Valleyview Health Office prior to the start of school as well as a physical. If your child does not turn 11 until after the start of school, a letter from your pediatrician with the date the vaccines are to be administered will be accepted.
